Location

1. Just by the entrance to Ancient Forest, behind the hill on the left after entering Mushroom Forest:

4. High up in a tree on the half-island with the Bees. Get there by going inside the giant mushroom, and then grab the jumping shoes, jump up the mushroom and then into the tree:
   
5. On the pole by the raft on the beach. If you stand on the fence and jump forward you can get it without any powerups:
